For researchers:  

1. Do you propose to undertake research as part of a program of study (i.e. Masters or PhD thesis)?

2. What kinds of research have you undertaken in the past?

3. Have you submitted a research proposal to your university/faculty/committee for approval? Please submit a copy of your proposal.

4. Has your project been approved or is it subject to approval by a Research Ethics Board at your educational institution or clinical setting?  If so, please attach application and approval.
